A borrower should submit this form to request a temporary payment reduction due to financial difficulty. This form must be completed in its entirety and include documentation of income and expenses for a one-month period. Reduced payments may be approved for periods not to exceed twelve months.
Loan Forgiveness Form
A borrower should submit this form to apply to repay a loan through loan forgiveness.
Medical Deferment Form
A borrower should submit this form when a medical condition makes it temporarily impossible to continue loan forgiveness or cash repayment. A deferment can last up to one year per occurrence.
Permanent Disability Form
A borrower should submit this form when a physical or mental disability, in the opinion of a medical doctor, makes it impossible for the borrower to work or earn money to satisfy repayment of the loan. This form is only used when a borrower will not be able to return to the work force in any productive capacity. This form may not be used to apply for disability discharge of federal loans.
A borrower should submit this form to request a temporary delay of loan forgiveness or cash repayment in accordance with the loan program's rules and regulations.
Residency/Postgraduate Training Deferment Form
A borrower should submit this form to request a temporary delay of repayment when required to complete a residency, internship, fellowship, or other postgraduate training program in order to practice his/her profession.
Student Data Form
A borrower should submit this form upon completion of his/her program of study. The information enables NCSEAA to maintain contact with the borrower throughout the life of the loan.
Verification of Student Enrollment Form
A borrower should have the registrar of the borrower’s school complete this form to be submitted with a Postponement/Deferment Request Form when a borrower wishes to postpone or delay loan forgiveness or cash repayment due to student enrollment.
