Postponement/Deferment Form (PDF, 1 page, 82 KB)

  • Suitable for: borrower who is unemployed, deployed to a military hot zone, or in school

If you are seeking a deferment for reasons of illness, family leave, or to complete a residency/fellowship/internship, you must also submit one of the following:

Borrowers with FFELP loans: FFELP loans are not eligible for deferment or forbearance once in default.

Financial Disclosure Statement (PDF, 2 pages, 553 KB)

A borrower should submit this form to request a temporary payment reduction due to financial difficulty. This form must be completed in its entirety and include documentation of income and expenses for a one-month period. Reduced payments may be approved for periods not to exceed twelve months.

Federal Financial Disclosure Statement (PDF, 5 pages, 1,011 KB)

Complete this form and return it to NCSEAA along with copies of your last two paycheck stubs (include spouse’s if married), and a copy of your most recent Federal tax return. If you have not filed a Federal tax return in the last two years, please include a signed letter of explanation and verification of your household size. This form can also be used for non-rehabilitation requests for a Reasonable and Affordable payment.

Loan Forgiveness Form (PDF, 1 page, 66KB)

A borrower should submit this form to apply to repay a loan through loan forgiveness.

NBCLP Refund Request Form (PDF, 1 page, 82 KB)

A borrower should submit this form after they have withdrawn from the National Boards Certification Loan Program and they want the funds returned to the NCSEAA to be applied to their loan.

Permanent Disability Form (PDF, 3 pages, 143 KB)

A borrower should submit this form when a physical or mental disability, in the opinion of a medical doctor, makes it impossible for the borrower to work or earn money to satisfy repayment of the loan. This form is only used when a borrower will not be able to return to the work force in any productive capacity.

Student Data Form (PDF, 1 page, 82 KB)

A borrower should submit this form upon completion of his/her program of study. The information enables NCSEAA to maintain contact with the borrower throughout the life of the loan.

Verification of Student Enrollment Form (PDF, 1 page. 61 KB)

A borrower should have the registrar of the borrower’s school complete this form to be submitted with a Postponement/Deferment Request Form when a borrower wishes to postpone or delay loan forgiveness or cash repayment due to student enrollment.

State Employees Voluntary Payroll Deduction Form (PDF, 1 page, 35 KB)

Suitable for: Borrowers who are State Employees and want to repay their loan through payroll deduction.

Family Educational Rights & Privacy Act (FERPA) Student Loan Information Release Form (PDF, 1 page, 77 KB)

A borrower should submit this form to authorize a person other than the borrower to speak with the North Carolina State Education Assistance Authority (NCSEAA) regarding the borrower’s student loan account.

Third-Party Authorization Form (PDF, 1 page, 500KB)

A borrower should submit this form to authorize a person other than the borrower to speak with the North Carolina State Education Assistance Authority (NCSEAA) regarding the borrower’s student loan account.

Aubrey Lee Brooks Foundation Scholarship Consent Form (PDF, 1 page, 102 KB)

An applicant should submit this form if they have been selected for consideration for the Aubrey Lee Brooks Scholarship. This form allows SEAA to submit an applicant’s information to the required members of the Aubrey Lee Brooks Foundation Scholarship selection committee.

If you are seeking a deferment for reasons of illness, family leave, or to complete a residency/fellowship/internship, you must submit two forms: a postponement/deferment form and a medical deferment form or residency/postgraduate training deferment form.
